One of my American friends has watched some of my DVD's from time to tiem, and I notice that the english subtitles are horendous. In some cases, entire lines of dialogue are left out just because they aren't weighty with respect to the plot (usually all the funny/interesting lines). Also, the word choice in some cases is below-average; I don't understand if they're just trying to be concise or actually trying to translate properly We're always mentioning audio/video quality, but these subtitles need some refinement, as well.

The grammar is often pretty poor and translations are often too literal and/or unnecessarily wordy. I've seen some horrendous subtitles, both on Indian DVDs and on Hong Kong films. There's a site that lists some funny subtitle faux pas from Hong Kong films, but I can't find it.
